Payment policy

Blossom Hearing Center (BHC) does not participate in insurance plans. Payment for hearing devices and services provided by BHC is the direct responsibility of the patient. BHC will provide you with an itemized receipt and CMS1500 form containing all information necessary for you to file for reimbursement from your insurance company for covered services. We suggest that you contact your insurance provider even before you first appointment to inquire about your hearing care coverage including out-of-network benefits. If you are unable to make that call, we can do it with you while you are at our office.

Payment is expected at the time of service and may be made by check, cash or credit card. Payment plans are available on amounts over $1500. Automatic payments can be arranged via credit cards.
